Our medical team provides a wide variety of integrated services to support your overall health and wellbeing.

We have medical and school-based clinics located across Kent County. We also offer primary medical care services for people experiencing homelessness and those exiting the justice system.

Accessibility is our mission. Live, on-site interpretation is available for all languages.

Overview

At Catherine’s, enhanced primary medical care is our specialty. As a patient-centered medical home, you are the leader of your care team, working with the guidance and expertise of our board-certified doctors, nurses, physician assistants, and nurse practitioners. Together, you and your provider will focus on treating your medical conditions and preventing problems from occurring in the future. From your first visit, we’ll work to understand your unique needs and preferences and set you on a path to better health.

We can help you with your routine care needs, treat unexpected health issues, or help you manage chronic conditions. We will also connect you with a specialist if needed.

Care for Chronic Conditions

Catherine’s uses a team-based approach to make living with a chronic condition such as diabetes or high blood pressure more manageable. As a patient of Catherine’s Health Center your primary care team will include behavioral health, care management, community health workers, onsite health coaching, and community partners to provide the best care for you.

Substance Use Disorder Treatment

We view care for substance use disorder the same as any other ongoing health concern, like high blood pressure or diabetes, including the use of medications to treat chronic conditions. Catherine’s follows a harm-reduction model and will work alongside you when you are ready to address substance use. We currently provide medication for opioid use and alcohol dependence. Behavioral health services are also available to provide support for patients making changes or experiencing stress and worries.

Infectious Disease Services

Catherine’s provides comprehensive care for patients living with or at risk for infectious diseases

  • Screening and treatment for hepatitis C
  • Screening and prevention (PrEP, PEP) services for HIV
  • Linkage to care for HIV treatment 
  • Screening and treatment for other sexually transmitted infections
