After Hours Care
If you are experiencing a medical emergency, call 911 or go to the nearest emergency room.
If you have an urgent need outside of business hours, please call your clinic’s main phone number and leave a detailed message with the best number to reach you. Our on-call provider will quickly return your call. Find your clinic phone number here.

You can use our online patient portal to request medication refills, request appointments, send messages to your provider, view lab results, and more.
Appointment Cancellation Policies
New Patients
It’s important that you attend your appointments because your health matters. But, we know life happens. If you need to reschedule an appointment, please call your clinic. If you are unable to make an appointment on time, or at all, please notify us 24 hours before your appointment. If you miss your first new patient appointment, you can contact us to reschedule. However, if you miss two new patient appointments, you will not be able to schedule again with us for one year.
Returning Patients
For returning patients, you are only able to miss four appointments per year before we ask that you find care elsewhere. Please ask our staff if you have more questions regarding this policy.
Patient Centered Medical Home
Catherine’s Health Center is a dedicated Patient Centered Medical Home. This means that you will always be at the center of our care team, and we will always strive to support your whole health.
At Catherine’s, we provide holistic healthcare in a family practice setting. Your primary care provider will be a doctor, nurse practitioner, or physician assistant, and they will lead your care team. You will be supported by nurses, medical assistants, and the full staff here at Catherine’s. We work together to make sure you can get and stay healthy. We believe science-based evidence is important, so we base our medical decisions on up-to-date research. We hold the highest standard of care for our patients.
Your health is at the center of our care team. Because good health results from an active partnership, we invite you to be a full participant in your care plan.
