What you owe when receiving services at Catherine’s Health Center depends on if you have health insurance and what your insurance covers. You can find more information below about insurances we accept and insurance assistance.
What Insurances Do We Accept?
We accept a wide variety of insurances, including Medicaid, Medicare, marketplace, and private insurance. If you don’t see your insurance here, please reach out! We’d still be happy to help find an affordable way to partner in your care.
About the Sliding Fee Discount Program
If you need help paying for health care expenses, we offer discounted fees based on your household size and income. This is called the sliding fee discount program.
How Do I Apply?
Fill out a Sliding Fee Application and bring it with you to your next appointment. Our front office staff can also help you with your application. Simply give us a call or ask our staff in person. To finish the application, we will need proof of income from the most recent year, which can be collected from any of the following:
- 1040 tax form
- W2 tax form
- Government assistance statement (SSI, disability, unemployment) 4506T
- Letter of Support (completed by anyone with whom you live that provides shelter and food)
- Paycheck stub

If you are experiencing financial hardship, please ask our staff about the Open Hearts Fund, a community funded program that helps our patients pay off their medical bills.

Good Faith Estimates
You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
Email us at billing@catherineshc.org to request your good faith estimate or ask any questions.
How to Pay Your Bill
In Person
Pay your bill in person at your clinic with cash, check, or credit card.
Phone
To pay your bill by phone, call (616) 483-0750, then press 6 for medical billing, or press 7 for dental billing.

You can choose to make a one-time payment or set up a recurring payment. Type in the Account Number found on your statement in the “Invoice #” text box.
