Income Qualifications

Catherine’s Health Center offers primary medical care to people who meet certain income guidelines. Our primary areas of geographic focus are the Creston and Belknap Neighborhoods and greater Grand Rapids, Michigan.

For uninsured people we offer assistance in applying for coverage under the Healthy Michigan Plan and in the Health Insurance Marketplace. Our partner in helping people find the right health insurance products is Enroll Michigan. We have several trained insurance navigators who offer Health Insurance Navigation services to help people find the health insurance plans that best fit their personal or family situations. A sliding fee scale is available for people who qualify. We require documentation of income for anyone requesting financial assistance.

Medicaid becomes unavailable in the state of Michigan at or above 138% of the Federal Poverty Level (FPL). The table below is provided as a reference for Health Insurance Navigation purposes. Please be sure to ask us if you have any questions.
