Ever since Catherine’s opened its doors in 1996, we’ve seen the dramatic impact that poverty can have on someone’s health. People living in poverty face multiple complex barriers to improving or sustaining their health, including a lack of access to quality care, an inability to navigate the healthcare system, low health literacy, and many other social determinants of health. When patients don’t have the necessary resources and support, they experience higher rates of cardiovascular disease and other chronic diseases. The cost burden often falls on emergency departments and the broader community, while these individuals and families experience a significant reduction in quality and years of life.
In order to address inequitable cardiovascular health outcomes among the underserved in our community, Catherine’s built Live Heart Smart, a robust suite of programs. Using direct patient input, Live Heart Smart was designed to engage patients as active participants in their care and to address social determinants of health.
Live Heart Smart is funded by the AstraZeneca HealthCare Foundation© Connections for Cardiovascular HealthSM program and has served 2,000+ individuals and families in West Michigan since its inception in 2012. Participants have experienced measurable improvement in weight and body mass index, systolic and diastolic blood pressure, cholesterol, glucose, and A1C, reduction in scores on depression, anxiety and substance use/abuse screening tools, and a decrease in annual emergency department visits.
In late 2018, Catherine’s began mentoring two newly-funded Connections for Cardiovascular HealthSM grantees. We’re working with the Asian Health Coalition in Chicago, Illinois and the Sisseton-Wahpeton Oyate of the Lake Traverse Reservation in the Northeastern part of South Dakota and a small portion of the southeastern corner of North Dakota. We’re honored to partner with these communities as they build and implement community-based, culturally appropriate programming to improve cardiovascular health outcomes in their own communities.
