Free Clinics of Michigan (FCOM) is a network of volunteer-staffed free clinics that provide health care services to the uninsured or medically under-served within the state of Michigan.
Exalta Health is a nonprofit, faith-based health center serving low-income families in West Michigan. They are located in the Burton Heights neighborhood of Southeast Grand Rapids.
The CNA is our neighborhood association and one of our closest partners. Their mission is “to organize neighborhood residents and resources to promote the social, economic and environmental health of the Creston community.”
Through its Activate West Michigan program, the YMCA seeks to improve the health and quality of life among West Michigan individuals, families, and communities by promoting healthy and active lifestyles.
Creston Community Gardens is a network of gardens within the Creston neighborhood. Their mission is “to provide residents and friends of the Creston neighborhood with access to community gardening; a green space to grow community and fresh, organic vegetables, fruits, and flowers close to home.”
New City Neighbors is a Creston neighborhood-based ministry who focuses on the renewal of the neighborhood through service and empowerment. Catherine’s is especially involved in their urban farming project.
The Family Outreach Center (FOC) is designed to provide quality outpatient mental health, substance use disorder, education, prevention and support services for children, youth and families living in the inner city of Grand Rapids, Michigan and all of Kent County.
MSU’s College of Human Medicine is committed to educating exemplary physicians and scholars, discovering and disseminating new knowledge, and providing service at home and abroad. Catherine’s is pleased to offer learning and service experiences for medical students and has valued relationships with several MSU faculty members.
The Health Department serves Kent County in which Catherine’s is located. The department is responsible for monitoring the health of the community and ensuring that certain services are accessible for its citizens. Catherine’s enjoys partnership arrangements with many of the Department’s programs and services and has a particularly close partnership involving its programs for women.
The MDCH is the state government department which oversees health policy management. Its mission is to “protect, preserve, and promote the health and safety of the people of Michigan with particular attention to providing for the needs of vulnerable and under-served populations.”
The State of Michigan’s Breast and Cervical Cancer Control Program provides low-income women with access to life-saving cancer screening services and follow-up care, including cancer treatment if that should be needed. The WISEWOMAN Program provides chronic disease risk factor screening and healthy lifestyle behavior support to Michigan women. See our page on Women’s Health Programs to learn more about how we partner with this program.
The Grand Rapids Community Foundation exists to pool and distribute resources across the Grand Rapids area to support philanthropy and strengthen the lives of Grand Rapidians.
Enroll Michigan is a statewide network of non-profit agencies providing free enrollment support services to health insurance consumers.
